Thank you for coming forward to volunteer with the Owen Sound Minor Soccer program this upcoming season. We rely totally on volunteers to deliver our programming and without people coming forward we would not be able to run a successful program. This application form will request information from you to complete your application as well as acknowledgement of forms that you will need to complete to volunteer with OSMSA. We will be requesting the following:

• Acknowledgement of Code of Conduct, House League Bylaws, Limits & Boundaries for OSMSA Volunteers, and Fair Play Code for Coaches.
• Information on Concussion Resources and Acknowledgement of Rowan's Law
